What are the benefits of blepharoplasty?

Lower eyelids usually involve repositioning of fat to achieve a youthful and less tired appearance. How do I prepare for blepharoplasty surgery?

You will be provided with a list of pre-op instructions. You will be required to stop taking aspirin or other blood thinning medications two weeks before surgery, as these can lead to issues with bleeding and clotting. In addition, you’ll need to arrange for transportation to and from the surgery.

What are the risks of blepharoplasty?

Dr. Frankel will perform your surgery safely and with the utmost care in order to obtain the best possible results. However, every surgery does have possible risks and complications. Blepharoplasty risks include bruising and swelling, numbness, infection, scarring, the inability to close your eyes, asymmetry, dry eyes, ectropion (lower eyelids that pull downward), loss of eyelashes or brow hairs, pain and light sensitivity, visual changes and blindness.

What is the recovery from blepharoplasty?

After blepharoplasty, you will have bruising and acute swelling that will typically last between one and three weeks. The results will continue to change and improve over many months or even a year. You may experience itching at the incision sites, which is a normal part of the healing process. This may last for a couple of weeks. Avoid rubbing or itching your eyelids until they are fully healed. Tearing, burning, tightness, itching, puffiness and red, bumpy incision lines are all normal, and should improve as healing progresses.

To prevent bleeding and increased swelling, avoid extreme physical activity, including athletic activities and intercourse for four days. After 24 hours, you are encouraged to go for light walks and strolls. Light cardio, such as an elliptical machine or spin bike on a low resistance can begin on post-operative day seven. Do not engage in any heavy cardiovascular activity (running), any activity that involves straining (weight lifting) or anything that involves bending over (yoga, pilates) for at least three weeks, unless otherwise instructed.

Will I have scars from blepharoplasty?

You will have very small scars from an upper blepharoplasty, but they should fade within four to six months. Dr. Frankel is very skilled at hiding the scars in the natural creases of the eyes, so they are not very noticable. You can also apply a specialized cream to help minimize scarring.

Can blepharoplasty be covered by insurance?

BROW LIFT

What is a brow lift?

A brow lift, also known as a forehead lift, provides a more relaxed and aesthetically pleasing appearance by repositioning tissues, rather than removing them. In this way, Dr. Frankel feels it is a conservative way to achieve a more harmonious balance to the face. The procedure is typically done with the use of a fiberoptic scope through small incisions within the scalp, so there are no scars.

Can I pair a brow lift with other facial plastic surgeries?

Patients often combine a brow lift with blepharoplasty or other virtually any other surgery. Talk to Dr. Frankel to see if you’re a good candidate for other procedures at the same time.

What happens during the surgery?

After the anesthesia is administered, several small incisions are made within the hairline. With the aid of a fiber optic endoscope, Dr. Frankel releases and then elevates and fixes the underlying muscle and tissue to help bring the brow back into an ideal position. Once in position, the brow is reattached to the bone and excess tissue is redistributed to other areas of the brow that could use filling. The muscles that control frowns may also be altered. The goal is to soften the signs of aging without raising the brow so much that the patient looks startled, and to not remove skin or tissue.

How long does brow lift surgery take?

Every surgery is different, but in general, the surgery may take two hours to complete.

Are there things I need to do to prepare for brow lift surgery?

It’s important to get plenty of rest so that your body can heal. Sleep with your head elevated to lessen swelling. Ice packs also help with swelling. In eight days you will return to your doctor’s office to have the staples removed. Expect some bruises and swelling. Swelling begins to subside after a couple days, and bruises fade after a week or two. Make sure you avoid strenuous activities while you heal. Something as simple as bending and lifting a heavy item can pull at the stitches, so follow the post-operative instructions regarding when you can resume your normal routine.

Are there risks or complications?

There is the chance of allergic reaction to anesthesia, infection, bruising and swelling, hematoma, hair loss, nerve injury, scarring, sub-conjunctival hemorrhage, pain, numbness and an adjusted hairline.

Will my insurance pay for brow lift surgery?

Brow lift surgery is typically considered cosmetic and elective, and is therefore, not covered by insurance. If the procedure is to correct damage following an accident or due to a medical condition, talk to your health insurance company or primary care doctor to see if coverage is offered.

BUCCAL FAT REMOVAL

What is buccal fat removal?

Buccal fat removal is a surgery to thin the cheeks. Buccal fat removal removes the buccal fat pad, which is located in the cheek at the level of where the upper and lower teeth come together.

How can I prepare for buccal fat removal surgery?

You will be provided with a list of pre-op instructions. You will be required to stop taking aspirin or other blood-thinning medications two weeks before surgery, as these can lead to issues with bleeding and clotting.

In addition, if you smoke, you must discontinue smoking at least three weeks prior to surgery. Cease all forms of nicotine, as well as vaping. Nicotine reduces blood flow to the skin, compromises healing, and jeopardizes a healthy result. If you have a history of cold sores then it is advisable that you begin taking Valtrex three days before the surgery.

What can I expect during recovery following buccal fat removal?

You will have sutures in your mouth, on the inside of both cheeks. Consume a liquid-only diet the day of surgery. Be sure to avoid salt. Your sutures are dissolvable, and should dissolve on their own within two weeks.

Healing is a process, and on days three and four, you may see an increased amount of swelling. The swelling will subside over the next few weeks. Brush your teeth gently. Absolutely no bending, lifting or straining for three weeks after surgery. Numbness, tingling, tightness and dull pain are normal, and should improve as healing progresses.

You may have a feeling of tightness and numbness, which is normal and may last two to three months. Full healing may take up to 18 months, as you are relying on your tissues to contract over time.

Will I be left with scars from buccal fat surgery?

Sutures will be on the inside of your mouth, so you will have no visible scarring.

What are the risks of buccal fat removal surgery?

DERMAL FILLERS & NEUROMODULATORS

What are aesthetic injections?

Aesthetic injections are used to treat fine lines and wrinkles and/or add volume to various parts of the face. These injectables fall into two general categories; fillers and neuromodulators. Dr. Frankel is a leading expert on utilizing injectables to treat a wide variety of issues and has published several original studies and lectured internationally on the subject. Injectables are commonly used to elevate the eyebrows or modify their shape and treat various wrinkles around the upper one third of the face. Other uses for aesthetic injections include lifting the corners of the mouth, filling in folds, elevating the cheeks, improving the jawline, rejuvenating the earlobes, eliminating dimples in the chin, and treating some types of scars. Many people choose injectable fillers as an alternative to invasive plastic surgery, or in combination with other procedures to enhance their rejuvenation results.

In addition to manufactured injectables, fat transfer may also be appropriate in situations where a large amount of volume is required. This involves harvesting fat cells from one part of the body and injecting that fat elsewhere. There are advantages and disadvantages to fat injections and Dr. Frankel will discuss this with you at your consultation.

What happens during the aesthetic injection process?

These treatments are done with the patient awake and participating in order to optimize the results. The face is cleaned and then injected with tiny needles to render the desired effect. Patients will feel the initial needle stick but then become numb as the anesthetic in the product begins to take effect. Cold compresses will be applied and the patient will leave with ice packs to apply. Often times a patient can go straight back to their office or to social functions but they should expect some swelling and potential bruising.

If a fat transfer is performed, the patient’s own fat will be harvested from other areas of the body, such as the thighs or abdomen, through liposuction first and this is performed in the operating room. The doctor will then inject that fat into the new locations, typically overfilling the area to account for re-absorption that will take place. The fat will settle, and you will experience the increased volume you desire.

How long does the procedure take?

Aesthetic injections are a very quick procedure, taking mere minutes after the prep-work has been completed. The number of injections will dictate the total time the procedure takes, though it is generally about 15 minutes.

What should I expect following an aesthetic injection?

Makeup can be applied, but not until the needle marks have close (approximately 30 mins) and patients are warned not to apply pressure to prevent movement of the filler. There may be some mild discomfort, which can easily be addressed with a cold compress. Any minor swelling or bruising will fade within a few days. There is no downtime with aesthetic injections that are performed as standalone procedures.

Are aesthetic injections permanent?

Aesthetic injections are not permanent and will need to be “touched up” at regular intervals. They do last longer today than they did years ago, but they do not last forever.

Each person metabolizes injectable fillers at a different rate. Some last a few months, others can last a few years.

Are aesthetic injections safe?

Aesthetic injections are very safe when performed by a trained physician. Over the years, since the practice of using injectables became popular, they have improved and the risk of allergy, adverse reactions and pain has lessened. Many types of current injectables, such as Restylane and Juvederm, are composed of substances that are found naturally in humans. Although the products themselves have become safer, the various applications of the fillers and the total number of people being injected throughout the world has resulted in complications such as skin necrosis and blindness. These complications are extremely rare and proper injection technique will decrease their likelihood, although not to zero. If you have concerns then please discuss with Dr. Frankel.

Those concerned about neuromodulators being poisonous due to their link with botulinum toxin should be aware that these drugs are purified derivatives and used in very low concentrations, making it perfectly safe for use by experts.

What risks are associated with aesthetic injections?

Aside from the typical minor swelling and bruising that can occur, there are other risks when using aesthetic injections. These may include allergic reaction, rejection of the filler material, acne flare-ups, asymmetry of facial features, bleeding, infection, death of the skin, skin rash, migration of the material away from the injection site, temporary numbness and temporary paralysis of the facial muscles.

Additional complications may arise with the use of neuromodulators such as headache, flu-like symptoms, nausea and drooping or weakness in the face. Botulism-like symptoms are very rare. They include trouble breathing, difficulty swallowing, slurred speech and muscle weakness.

FACELIFT

What is a facelift?

A face lift, also known as a rhytidectomy, is a popular cosmetic surgery that is used to address several different signs of aging at once to rejuvenate the face and neck. What is a SMAS facelift?

SMAS stands for superficial muscular aponeurotic system, which is the layer of tissue that can be found in the top of the deeper tissues and structures of the face and neck. The SMAS facelift is an older technique that addresses the lower two-thirds of the face. It is effective in resolving sagging skin, excess fat and volume loss in the cheek area.

What is a mini facelift?

The term ‘mini facelift’ is poorly defined and without any specific anatomic meaning. However, it generally implies that less dissection is performed and a shorter incision can be utilized compared to regular facelifts.

What is a midface lift?

The purpose of the midface lift is to address sagging in the upper part of the cheeks and rejuvenate the midface region by redistributing volume vertically. Dr. Frankel uses a technique where there are no visible incisions and that provides for natural looking and ‘soft’ changes to the facial shape as a whole. How do you prepare for a facelift?

Most patients require a pre operative visit with an internist to ensure that there are no contraindications to surgery. Although patients will be able to be productive getting deskwork done at home, they should plan to be out of social situations for roughly two weeks. Most of the time Dr. Frankel will request that patients refrain from vigorous exercise for three weeks.

What is recovery like for a facelift?

Patients should expect swelling and bruising for approximately two weeks after a facelift. Although they may experience some pain, most people state it’s more uncomfortable and improves daily after the procedure. Pain medication is prescribed by Dr. Frankel in the event it’s needed.

Patients are encouraged to walk almost right away. The head bandage that Dr. Frankel will place over the incisions after surgery will likely be removed within three days following the procedure, and sutures and surgical staples are usually removed between eight and nine days after the procedure.

Patients usually find that their swelling and bruising is reduced enough that they can run errands with just a pair of sunglasses within 10 days. For more intimate encounters, such as dinner parties or coffee with an old friend, most patients may prefer to wait two to three weeks, to be sure the signs of surgery are almost entirely gone. By that point, most patients have no more than some discoloration remaining, which can be easily covered with makeup. Most patients are also able to return to work a few weeks after surgery. It’s important to keep in mind that every patient is different, so recovery time varies.

What are the possible side effects or risks of a facelift?

What is the best age for a facelift?

Will there be visible scars after facelift surgery?

Facelift surgery involves creating incisions. These are placed so that they are as hidden as possible and a great deal of attention is directed towards minimizing their appearance. When the sutures are removed the incisions may be pink in color. These scars will lighten over the next few months, and become less prominent as the skin heals. Since most of the scars will be hidden well under the hairline or with the natural creases of the skin, they are barely noticeable. Occasionally patients have more visible scars due to color or texture or thickness and there are several things that can be done to improve this should this present a problem. As a rule, scars rarely present issues for patients. By taking excellent care of their skin and reducing their exposure to the sun, patients can significantly reduce the appearance of their scars.

How long will my facelift results last?

Unlike fillers, which normally disappear within a year, the benefits of a facelift are permanent, so your facelift will give you lifelong results. It’s important to remember, however, that you will continue to age normally after your facelift and you won’t look exactly the same 20 years later than you do ten years after the surgery. For the longest-lasting facial rejuvenation, it’s important to maintain normal preventative measures like proper skincare, sun protection and nutrition.

Will my face have that tight look afterward?

Dr. Frankel uses surgical techniques that rely on the deeper facial tissues for suspension so the skin does not show lines of tension and look pulled. Additionally, Dr. Frankel prefers a vertical vector for his lifts and this mitigates having the ‘windswept’ look and having the mouth become distorted. There are certain situations where a patient might be at risk for looking pulled and if that is the case, Dr. Frankel will discuss that before moving ahead with the surgery.

Are there any non-invasive alternatives to a facelift?

Skin treatments, such as Ulthera and Thermage, are designed to tighten the skin and Dr. Frankel feels they can produce modest improvements and occasionally more significant results. However, the degree of tightening from such therapies is not comparable to what can be accomplished surgically.

Laser treatments and peels can improve skin texture, and injectable treatments, like fillers and neurotoxins, can reduce fine lines and wrinkling, but they are all temporary, and none can provide results as dramatic as a facelift. If you seek significant, permanent results, there is no true alternative to a surgical face lift. Some procedures may complement a facelift, but they cannot replace this effective surgical procedure.

JAWLINE CONTOURING

What is Jawline Contouring?

A procedure or combination of treatments to enhance and define the jawline. Dr. Frankel considers four options when he is assessing an individual and how he might improve the overall aesthetics of this vital anatomic structure; Chin Augmentation, Submental Lipo, Platysma Plication And Fillers & Neuromodulators.

What is chin augmentation?

Chin augmentation is a procedure designed for patients with a small (under projected) or short chin, which makes the face appear out of balance with the rest of the face. Surgery involves placing an implant through a small incision under the chin and the downtime is minimal. Occasionally, Dr. Frankel may suggest injecting filler to the chin in an attempt to simulate what the surgery would accomplish or to make a subtle improvement without the need for surgery. Filler is not quite the same as an implant, but sometimes it can suffice and give you an idea of the effect.

What is Submental Lipo?

This removes subcutaneous fat from beneath the chin and mandible to reveal a more defined chin and jawline. With the fat no longer present, the underlying bony structure becomes emphasized. Liposuction is done with a hollow cannula and carries very little risk of complications. It can be performed alone but is frequently combined with other procedures.

What Is Platysma Plication?

The platysma muscles are two paired superficial muscles that often obscure the angle between the jaw and neck. The muscles can also be seen as vertical ‘bands’ sagging under the chin and creating a ‘Turkey Neck’. These muscles can be approached from a small incision under the chin and modified in a number of ways to improve the contour of the jawline. Surgery on the platysma muscles requires that the patient wear a compressive dressing for several days following the operation.

How can fillers & neuromodulators improve a jawline?

In people who are developing laxity to their lower facial soft tissues and beginning to notice subtle jowls forming, injections of filler can be very helpful. Additionally, people with otherwise narrow mandibles or chins can also benefit from targeted placement of volume along the jaw. Dr. Frankel has proven that Neuromodulators can improve chin aesthetics and published a paper and lectured on this topic. And injections to the platysma muscles can also create better definition of the edge of the jaw as well as lift the soft tissues slightly.

What options do I have for implants?

Chin implants are crafted from solid silicone. They are firm rubber, but they do not lose their shape over time. These implants come in a variety of shapes and sizes, and this makes it easy to find the perfect framing for your chin’s new look.

What will the doctor discuss during the consultation?

During the consultation, Dr. Frankel will look at your bone structure and see if you’re a good candidate for chin augmentation. If you are, he will discuss the different sizes and shapes of the implants and the different options available to you and use the computer to simulate your desired outcome. You’ll also discuss things you must do before and after the surgery, what happens during the surgery, and potential complications or risks.

What happens during the surgery?

Chin augmentation is a very safe procedure with low risk. Following the surgery, there may be some bruising and swelling, scarring, inflammation, skin sensitivity, nerve injury or loss of sensation in the chin. Infection is another risk. If an infection sets in, the doctor may have to remove the implant. Finally, there is the chance that the implant will shift while your body heals.

Does insurance cover chin augmentation?

Chin augmentation is an aesthetic surgery, so medical insurance does not usually pay for the procedure. There are exceptions to the rule, usually in cases where the surgery is to repair damage caused by trauma, like a car accident. LIP LIFT

What is a lip lift?

A lip lift is a surgical approach to improve the aesthetics of the face and mouth. The procedure involves removing some tissue from under the nose, and thus shortening and everting the upper lip. The outcome improves the balance of the face, and creates a fuller, more youthful-looking mouth. Lip lift surgery takes approximately 90 minutes, is done under local anesthesia, and not associated with a painful recovery. Not everyone is a candidate for this procedure and Dr. Frankel carefully weighs the pros and cons before making a decision.

What is a neck lift?

Both issues are correctable and creativity is employed to render the best outcome. Otoplasty usually involves incisions created at the back side of the ear, so they are not noticeable.

Are there age restrictions associated with otoplasty?

Otoplasty is commonly performed on small children. Patients must be at least 5 years old, at which point the ears have fully developed. Performing the surgery prior to starting school can benefit children with prominent ears by eliminating the source of teasing from others. SKIN REJUVENATION

What happens during skin rejuvenation?

During a skin rejuvenation procedure, the physician uses a laser to send short, concentrated pulsating beams of light to the skin. The light allows for excellent precision, so only the damaged skin receives treatment. The irregular skin is removed, one layer at a time by the laser. While the outer layer of skin, the epidermis, is being removed, the lower lying layers, called the dermis, are being heated. The warmth stimulates the growth of collagen fibers, which are the proteins that give your skin its firmness and elasticity. The new skin that forms will be firmer, smoother and healthier.

How long does the procedure take?

Most patients can expect their treatment to take between 30 minutes and two hours. Times vary based on the size of the area of skin being treated and the extent of the skin damage.

Laser skin rejuvenation can be painful. To control the discomfort, the doctor will apply a local anesthetic to numb the skin. Some patients receive sedation to keep them at ease. In some cases, patients have laser skin rejuvenation performed simultaneously with other plastic surgery procedures.

Following the procedure, pain medication can be prescribed by the doctor to help manage discomfort that may persist afterward. Most patients feel the sensation is similar to mild sunburn.

Where is laser skin rejuvenation performed?

Your procedure will be performed as an outpatient procedure at the Lasky Laser Center, located adjacent to Dr. Frankel’s office. The most aggressive laser treatments, or those that require sedation, are personally performed by Dr. Frankel at the Lasky Clinic.

How do I prepare for laser skin rejuvenation?

Six weeks or more before the day of the procedure, skin treatments may be prescribed to prepare your skin and ‘prime it’ for laser skin rejuvenation. Skincare regimens are individualized to each patient’s skin type to properly ensure optimal results and limit complications.

If you smoke, you must discontinue smoking at least three weeks prior to your procedure. Cease ALL forms of nicotine, as well as vaping. Nicotine reduces blood flow to the skin, compromises healing, and jeopardizes a healthy result.

What can I expect following laser skin rejuvenation?

Special dressings will be applied. Dressings may need to be changed and/or topical treatments may need to be applied to aid healing. You will experience some redness and swelling. Some people also report itching. If the skin begins to ooze and crust over, do not pick at it. Specific instructions will be given on how to clean the area.

After five to seven days, the skin will become dry and begin to peel. The new skin will be pink, but will begin to gradually return to your normal skin tone within two to three months. Makeup can be applied as camouflage after the area has healed.

How long does it take to make a full recovery?

It can take up to one year on some patients for the pinkness in the new skin to completely return to normal. It is important to continue to care for your skin and practice excellent sun protection.

What risks are associated with laser skin rejuvenation?

The following complications may arise as a result of laser skin resurfacing. Contact Dr. Frankel if you experience any of the following: acne flare ups, hyperpigmentation (darkening of the skin), bacterial infection, reactivation of cold sores, excessive swelling or small white bumps (milia). Most side effects can be treated. Scarring, though rare, can also occur, especially if the skin is picked at while healing.

Will my insurance cover the procedure?

Most health insurance companies will not cover elective plastic surgery procedures, unless they are medically necessary. Speak to your insurance company to find out what may be covered.
