While rhinoplasty surgery is often the only way to create the changes that someone wants, there are several non-surgical procedures that Dr. Frankel commonly employs to achieve a patient’s goals. The nose shape can be modified with injectable fillers and/or Botox. Soft tissue swelling and thickness can be diminished with cortisone or other such injections, and nasal skin texture and pore size can be improved with Allumera or photodynamic therapy or lasers. Finally, uneven pigmentation or color can be treated effectively with lasers. Certain causes of nasal airway obstruction can also be treated non-surgically by placement of a bio-absorbable support called Latera.
These procedures have minimal healing times and have immediate or visible results within days. They are also minimally invasive, do not require any anesthesia, and can be performed easily in Dr. Frankel’s office.

Non Surgical Procedures
Nasal Shape
There are certain aesthetic issues that lend themselves to modification by the addition of volume to strategically placed areas within the nose. The tip can be made to appear more turned up, the bridge can often be made to look more straight and symmetric, and a hump can be diminished or camouflaged altogether. Tip aesthetics are less safe and easy to improve with filler, but there are specific instances where filler can be helpful as well. The use of a neurotoxin, such as Botox, can also help to lift the tip, lower the height of the nostrils and even narrow the nose at times by weakening specific muscles.
Dr. Frankel will have a sense of how impactful these options are once he examines your nose.
Nasal Soft Tissue Envelope
The appearance of the nose is not only dependent upon the underlying bone and cartilage, but also on the quality of its covering. Many people have overactive sebaceous glands that create increase pore size, oiliness and thickness. These issues can be improved with non-surgical treatments using a chemical that becomes activated by a special light, and oral medications can occasionally be helpful.
For patients who have had previous surgery and developed scar tissue under their skin, cortisone injections can be used to thin that out and hasten the exit of edema remaining within the nose. While the effects of such modalities are limited, they can be the difference between a mediocre result and an excellent one.
Nasal Skin Tone
The skin over the nose might have areas of irregular pigmentation that can be improved with topical products or laser treatments. It is not uncommon for patients who have had multiple prior surgeries to have redness or even bluish discoloration due to vascular changes. Such color variation can be distracting to an otherwise pleasing result, and treatment with vascular lasers can be very helpful.
Dr. Frankel has access to a variety of technologies, so each patient can be treated in a customized fashion.
Nasal Function
There are times when a person has difficulty breathing, but otherwise likes the look of his/her nose. In such situations, there may be things that can be done non-surgically to improve breathing without altering the shape. For cases where the issue is with poor support to the nasal sidewall, Dr. Frankel can place a bio-absorbable implant (Latera™) within the nose in just a few minutes. This device provides increased stiffness without adding any width, and has been proven to be effective.
Occasionally, the problem has to do with swelling inside of the nose, and this can be handled by applying radiofrequency energy to shrink the turbinates. Dr. Frankel is double board certified in Otolaryngology-Head and Neck Surgery, as well as Facial Plastic and Reconstructive Surgery, and is aware of how to best manage the nasal airway and functional problems.
