Consultation and Planning
After learning your specific desires and goals, Dr. Frankel will examine you at your initial consultation to determine if you are a good candidate for surgery. Dr. Frankel prefers to reach the desired outcome with the least amount of risk and invasiveness possible, so if non-surgical treatments can achieve your goal then he will suggest them. The risks and benefits of a non-surgical approach versus a surgical one will be discussed and realistic goals and expectations set. Dr. Frankel individualizes his approach to every patient and he will invest the time to fully understand your desires, motivations and fears so that the two of you can come up with an optimal plan. You will be encouraged to ask him any questions you might have so that you feel fully informed.
Surgical Process
Depending on the surgery and conditions specific to each patient, a procedure may be performed with local anesthesia, IV sedation or general anesthesia.
The great majority of Dr. Frankel’s operations are performed under “twilight” anesthesia, where the patient is unconscious, but still breathing on his/her own. Anesthesia is administered by a board-certified anesthesiologist, and usually employs propofol.
Dr. Frankel will discuss what to expect in detail, from pre-operative instructions to the actual surgery to post-operative expectations and timelines.
Other procedures
Face Lift
Face lift surgery is the ultimate form of facial rejuvenation. Unlike lasers, topical treatments or injectable fillers, face lift procedures actually reposition the deeper structures of the face. By tightening these structures, the contour is improved and the skin appears smoother. A face lift can help create a firm, even appearance along the jawline, cheeks, jowls and neck, and sets the foundation for other, more subtle rejuvenative procedures.
Neck Lift
A neck lift is a cosmetic procedure that improves the appearance of the neck without the need for the incisions associated with a full face lift. Sometimes incisions are placed behind the ears, and sometimes a small incision under the chin is all that is necessary to achieve a more youthful appearing neck. Dr. Frankel will discuss whether or not you are a candidate for this more limited procedure at your consultation.
Blepharoplasty
Blepharoplasty is a surgical technique for improving eyelid aesthetics. While there is great variability to the specifics of what is done technically, most upper eyelids involve removal of some skin and reshaping of the underlying tissues to attain the most desirable and youthful eyes. Lower eyelids usually involve repositioning of fat to achieve a youthful and less tired appearance. Using precise techniques and tiny incisions, Dr. Frankel achieves natural and youthful-looking results without noticeable scarring.
Midface Lift
A midface lift is employed when there is a need to elevate and restore volume to the higher areas of the face, such as the cheeks. It can be a very powerful, yet subtle form of rejuvenating a face or for creating a more balanced and harmonious facial contour. The procedure is done on both young and older patients, and does not create any visible scars. Midface lifts can be performed as stand-alone procedures, but are more often done in conjunction with another facial surgery.
Brow Lift
A brow lift, also known as a forehead lift, provides a more relaxed and aesthetically pleasing appearance by repositioning tissues, rather than removing them. In this way, Dr. Frankel feels it is a conservative way to achieve a more harmonious balance to the face. The procedure is typically done with the use of a fiber optic scope through small incisions within the scalp, so there are no scars.
Otoplasty
Otoplasty is the surgical treatment for protruding ears. There are generally two reasons for protruding ears: a lack of formation of the anti-helical fold and an excessively deep cup of the ear called the conchal bowl. Both issues are correctable and creativity is employed to render the best outcome. Otoplasty usually involves incisions created at the back side of the ear, so they are not noticeable.
Lip Lift
A lip lift is a surgical approach to improve the aesthetics of the face and mouth. Dr. Frankel believes a lip lift can have the largest impact of all the procedures he performs. The procedure involves removing some tissue from under the nose, and thus shortening and everting the upper lip. The outcome creates improved balance to the face, and a fuller, more youthful-looking mouth. Lip lift surgery takes approximately 90 minutes, is done under local anesthesia, and not associated with a painful recovery. Not everyone is a candidate for this procedure and Dr. Frankel weighs carefully the pros and cons before making a decision.
Chin Augmentation
Chin augmentation is a procedure designed for patients with a small (under projected) or short chin, which makes the face appear out of balance, especially from a side view. Surgery involves placing an implant through a small incision under the chin and the downtime is minimal. Occasionally, Dr. Frankel will suggest injecting filler to the chin in an attempt to simulate what the surgery would accomplish or to make a subtle improvement without the need for surgery. Filler is not quite the same as an implant, but sometimes it can suffice.
Other procedures
Primary Rhinoplasty
Primary rhinoplasty is the best opportunity to deliver optimal results. Because it is the very first nasal surgery, the anatomy is undisturbed, and there is no scar tissue or missing cartilage. Careful planning and excellent execution can deliver gorgeous outcomes with fewer limitations.
Revision Rhinoplasty
Revision rhinoplasty is a specialized facial plastic reconstructive surgery to correct inferior results. Dr. Frankel can create a beautiful, natural-looking result that rebuilds support and structure of the nose for improved function and long-lasting results.
Non Surgical Treatments
As a pioneer and lead researcher in the field of injectables, Dr. Frankel is experienced and able to determine if a patient’s requests can be satisfied without the need for surgery. Philosophically, he believes it’s best to reach the optimal result with the most conservative approach.
